Make sure to print clearly and fill out the form completly. (2) Next, select the item that you want, the type, and the quantity. (3) Then, put the completed form and your payment in an envelope addressed to: Distant Thunder Films · 2850 Johnson Ave. Alameda, CA 94501. Make sure you wrap any checks or cash in a couple sheets of white paper so no one can see your payment threw the envelope. Distant Thunder Films is not liable for any lost payments. (4) You will recieve your order within 2-3 weeks of our recieving your order. PAYMENT: We accept cash, personal checks, travelers checks, and money orders.
