[1] Include the following in your package: head shots, bio, list of projects you've worked on, abilities and skills, acting education(if applies), the question sheet, and this form.
[2] Make sure that all of these are complete and correct. Do not include false information in submitions. Also, make sure that the entire package is complete with all parts of information included.
[3] Put all contents into large envelope. Make sure the correct amount of postage is acounted for.
